What person-centred care really means

Person-centred care means seeing the person first and the task second. Instead of fitting people into routines that suit the service, it starts with who someone is — their history, their relationships, what they value and how they want to live — and shapes support around that. It treats people as equal partners in their own care, with the right to make choices, take positive risks and be treated with dignity, whatever their age, condition or ability. The same diagnosis can mean very different things to two different people, so person-centred care resists one-size-fits-all routines and asks, every time, what matters to this particular person today.

It sounds simple, but doing it well takes real skill: listening properly, noticing what matters to someone, involving families without taking over, and balancing choice with safety.
